• Select “Account”, then “Payment and Subscriptions” and then “Redeem Codes”.

On a PS4 console: 

• Go to the PlayStation Store.
• Scroll down to the bottom of the sidebar and select “Redeem Codes”.
• Enter your 12-digit code and click "Redeem".
